I did face this issue in one of our development environment where the point Neo mentioned did happen. My scenario was we had some GL Levels associated to User Role, and this user role was having some set of User menu's attached and this user role was attached to a specific user. So this user when you goes to any associated menu link he would receive this NO_USER_ACCESS issue.
Well tried a lot, finally I deleted all the objects i.e. GL_LEVEL, USER ROLE, and user too and recreated them and reassociated each other and it resolved the issue. Please note that I didn't delete or edit the menu and its associated screensets. So you need to check with GL_LEVEL, USER ROLE and USER.
