Hi Ed,
Did you modify any planning parameters or are you using the default one shipped with OTM? There is a planning parameter FULL SHIPMENT CAPACITY PERCENTAGE THRESHOLD it should be 100.
Are you building shipments via Ground Schedules? If so, when you create the shipment, under the Equipment tab, you need to uncheck Ignore Equipment Capacity.
Can you also post screen captures of the following:
1) Itinerary Lane configuration
2) Equipment Configuration
3) Order Release (ship unit and line item)
4) Shipment
