Is is possible to add a workflow to OTM to request approval for expedited shipments?

Not sure if I got the question right. But if you are asking whether such scenarios can be modeled in OTM, the answer is Yes. You could use automation agents, external status and saved queries to flag such shipments for approval requests/actions.
