Adding Fields in Service Provider Manager Layout

Hi,

How to add fields in the Manager Layout of Service Provider.
For adding additional fields, type is required(Refnum_Value/Remark_text).
But this is not available in the manager Layout of Service Provider.
So what is the other alternative to add new fields to the manager layout
of service provider?
